Welcome!
We want to get off to a good start. It is my sincere hope that we can be of help to you. But first we need your help. Would you take some time to complete two questionnaires before we meet? This will help me be prepared to meet your needs. I can give some time prior to our meeting to think about you, what you want to deal with, and how best to help you.
First, if you will complete the Identifying Information questionnaire, the Personal Inventory questionnaire, and the 3 Minute Screening Tool I will get to know you a little. This will help me think about who you are, how you feel, and how well you are doing. Click on the link below and it will take you right to the questionnaires. As you complete them they will go right to me. I will let you know I got them and am reviewing them prior to our time together.
The material for each of the questionnaires is completely confidential and comes only to me. If you choose not to complete any questionnaire due to time or personal reasons don't let that prevent you from coming in or seeking help. However, if you do complete the questionnaires it will save us time and you money by letting us get off to a faster start.
In addition, if you want to print off the Intake Forms and complete them before you come to your first session you can bring them with you and that will also save us some time the first session. Otherwise you can just complete them at the beginning of your session. They may also answer some of your questions about how we will work together.

If you are a parent and are calling and coming for help with a child, please complete the additional questionnaire Child and Adolescent History. This will also come directly to me.
